Support Coordination can help you understand your NDIS plan, connect with the right services and make more confident decisions about what comes next.
Support Coordination is about helping you understand the options available within your NDIS plan and connecting you with the people, services and opportunities that fit your needs.
It can also help when things change, when you need to find a new provider, or when different parts of your plan need to work together more smoothly.
We can help break down your plan so you know what is funded, what options are available and where to start.
We can help you compare providers, services and other opportunities that may suit your needs and goals.
You stay in control of the decisions. Our role is to give you information, explain your options and help you move forward with confidence.
We can help coordinate communication between providers, family members and other professionals involved in your life.
Over time, Support Coordination can help you feel more comfortable making decisions, asking questions and managing more independently.
There can be a lot to keep track of when you have different providers, appointments, goals and plan requirements all happening at once.
Support Coordination can help bring some structure to that.
We can help you understand what needs attention, work through issues as they come up and connect the different parts of your plan so things feel more manageable.
Your needs, providers and priorities may change over time, and your Support Coordination should be able to move with you.
Depending on your situation, we can help with:
