NDIS Referrals

Make a referral to True Colours

Know someone who may be a good fit for True Colours? Send through an NDIS referral and tell us a little about the participant, what they need and what’s important to them.

Our team works across Brisbane, North Brisbane, Moreton Bay and Bribie Island.

A referral is just the beginning

More than just a name on a plan

A referral form can tell us what services are needed, but it rarely tells us the whole story. Once we receive an NDIS referral, we want to understand the participant’s routines, preferences, goals, personality and any specific considerations that may affect the way services are delivered.

That gives us a much clearer picture of what is needed and helps us consider the people within our team who may be the right fit.

A considered approach

Making the right match

Finding an appropriate staff member is an important part of the True Colours approach.

We consider the participant as an individual, including how they communicate, what they are comfortable with, their routines and the type of person they may work well with. Our background across frontline disability work, case management, Residential Youth Support, Child Protection and complex needs also means we understand that some referrals require a little more thought before services begin.

If there are behavioural needs, risks, existing plans or other important details we should know about, include them in the referral or speak with our team directly.
